ADRC DAP Disability Services Navigators help individuals and families navigate the disability services system and connect with supports that fit their needs. Every contact begins with Information and Assistance, where staff help identify needs, explore goals, and connect you with the right resources. Many questions can be resolved quickly, such as completing a Medicaid application or locating specific services, often within one or two visits. When referrals are needed, staff provide warm handoffs to ensure a smooth connection to other providers.

If you would like more ongoing support, you can choose to work with staff through Options Counseling, a short-term process focused on developing an action plan and identifying the steps and supports needed to reach your goals.

For some individuals, Service Coordination for Access to Long-Term Services and Supports (LTSS) may also be an option. LTSS coordination is provided when the Disability Access Point authorizes LTSS funding for a service. The service coordinator helps ensure that authorized support continues to meet the individual’s needs and goals over time. Eligibility for LTSS funding depends on several factors and is determined based on each person’s unique circumstances. It is a process that takes time and careful review, and our staff will work with you to understand your situation and what supports may be available.

The best way to learn whether LTSS may apply to you is to start with Information and Assistance. Staff will gather information, explain the process, and help guide you through next steps.
